Does Well N' Wise accept insurance?
Well N Wise does not participate in any insurance plans. However, upon request, a superbill can be provided to you which can be submitted for reimbursement to your insurance carrier.
What forms of payment are accepted?
All credit, debit, Venmo, Zelle, Health Savings Accounts (HSAs), as well as cash and personal checks are accepted.
Payments made with credit or debit cards are subject to 3.5% processing fees.
What are the fees?
Initial consultation takes approximately an hour and a half. Follow-up appointments are 30 to 45 minutes in length, depending on our goals and strategies determined during your initial consult. Consultations and follow-up appointments are conducted either in-person, on the phone, or via Zoom or Google Meet.
Well N' Wise structures it's fees around its' program structure which consists of 3, 6, or 12 month commitment.
Fees for service is approximately $108.00 per hour.
What is the cancellation policy?
Appointments that are cancelled less than 48 hours are subject to a $75.00 fee.
What information do I need to provide to get started?
After we confirm your appointment, you will receive an email containing forms that will need to be reviewed and completed. These include an extensive health history, a credit card authorization form, food log, informed consent, and a welcome letter.
Do I need a referral for Nutritional Counseling?
Physician referrals are welcome but are only required if your insurance plan requires a doctor referral for nutrition counseling/medical nutrition therapy. This is a case for Medicare which requires a referral from a primary care physician. Referrals may be sent to Diane's email address, diane@wellnwise.com. All medical information will remain confidential.
